BENEFITS SUMMARY

Agnew Associates offers an excellent benefits package, including medical and dental coverage, a life and disability insurance plan, and education incentives. Regular full-time employees are eligible to participate in most plans on their first day of employment with a few exceptions as noted throughout. Employees' benefits are provided entirely (100%) at company expense. Agnew Associates will pay half of the cost of family coverage.

Health Plan
The plan consist of a Preferred Provider Option (PPO) or Non-Preferred Provider Option, which has a Managed Prescription Drug Program with retail and mail-order components, and a Managed Mental Health and Chemical Dependency (MHCD) Program.

Reasonable and customary medical charges are normally reimbursed at 90% for PPO and 80% for non-PPO. The deductible for all covered charges incurred in a year is $300 for individuals with a $1,500 out of pocket maximum, and $600 for families with a $3,000 out-of-pocket maximum.

Adoption Assistance is provided for qualifying adoption expenses up to $1,000 and is not subject to a deductible.

LabOne’s Lab Card Program provides free outpatient laboratory testing with no deductible and no co-payment.

Benefits begin 30 days after employment date.

Dental Plan
The plan pays 80% for diagnostic, preventive, and most basic restoration expenses. Dentures, crowns, bridges, onlays, and orthodontic expenses are covered at 50%. A $50 deductible for individuals and a $150 deductible for families are subject to all covered dental expenses incurred in a year with a $1,500 annual dental maximum (excluding orthodontic expenses). Orthodontic expenses have a $1,500 lifetime dental maximum.

Benefits begin 30 days after employment date.

Audio-Visual Plan
The plan is available to aid in the expenses of audiometric examinations, hearing aid services and supplies, and eye examinations. The standard eye examination is covered in full, less a $25 co-payment. An annual allowance up to $105 will be provided toward the contact lens evaluation examination, fitting cost, and materials. Allowances for lenses are provided every year and a $50 allowance every other year is provided for frames.

Sick Benefit Plan
Sick leave is accumulated each month at the rate of one half-day per month worked for a total of six days per year. Sick leave may accumulate to a total of 30 days, and all of the accumulated account is paid when you retire at age 65 or thereafter.

Statutory Benefit Plan
Workers Compensation Insurance provides protection against accidental injury that occurs while performing the business of the company. It also insures against loss of time caused by any illness that occurs as a direct result of the company. These benefits include weekly disability benefits, payment of medical, surgical, and hospital benefits, and death benefits. The entire cost of this protection is paid in full by the company.

All employees are covered under FICA (Social Security) retirement benefits. To pay for these benefits, both you and the company pay Social Security taxes in equal amounts.

As required by state and federal laws, all employees are covered by unemployment insurance. The entire cost is paid by the company.

Long-Term Disability Plan
You will receive 66 2/3% of your monthly base pay beginning six months from the date of the disability up to a maximum of $8,000 per month. All benefits are rounded to the nearest one dollar. For a period of disability less than one month, 1/30 of the monthly benefit will be paid for each day of total disability.

Life Insurance Plan
You will be provided life insurance coverage of 200% of your basic annual salary subject to a $50,000 maximum.

Survivor Income Benefit Plan
This plan provides a monthly income amount to your survivor if you die while covered under this benefit. This amount is subject to your basic annual earnings.

Dependent Life Benefit Plan
A life benefit will be paid in the following amounts if your dependent dies while covered under this plan: Spouse- $2,500; Child (14 days old- 6 months)- $250; Child (6 months +)- $1,000.

Accidental Death/Dismemberment Plan
With this plan, you will receive 100% of your basic life benefit.

All life insurance plans are subject to age restrictions and limitations. The coverage with respect to the insured person’s dependents will be applicable only if the person is eligible, has enrolled, and is insured for such coverage.

401k Plan
After one year of service or 1,000 hours, you are eligible to participate in the company’s 401k Plan. This plan is not mandatory, but a benefit to help employees prepare for retirement. The company matches investments based on annual profits. A maximum of 15% of your annual income may be invested, with a maximum of $10,500 per year.

Holidays
The following paid holidays are observed: New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas Day.

Vacation Plan
Vacation is accrued at the rate of eight hours for every month worked for a total of 12 days per year. Vacation is credited on the same day of each month that employment began. You may accrue a maximum of 1 ½ times your annual vacation and you may borrow up to 24 hours of unearned vacation time.

Family/Personal Leave
With one or more years of service, you are entitled to take a leave of absence for up to 12 workweeks' duration in any 12-month period for the following reasons: birth, adoption, serious illness of a family member, or military leave. Leaves for other personal reasons or extended family leaves require company approval. The maximum length of time that will be approved for a leave of absence is 12 months.

Educational Development Plan
In order to develop and maintain the high level of expertise and professionalism to which the company strives, staff are encouraged to participate in professional and educational activities. The company recognizes that your professional effectiveness depends on your efforts to increase your knowledge and ability. The company will make every reasonable attempt to adjust your work assignments to permit attendance at seminars and training sessions that can help you remain knowledgeable of the rapidly expanding technology in your field.

If you wish to pursue an academic degree program, the company offers flexibility in scheduling and under special circumstances will aid in the financial burden of the expenses.

Professional Licensing Plan
The company encourages its professional staff to obtain their professional engineering registrations as soon as they are qualified to do so, regardless of their present assignment or level of responsibility within the firm. Employees are provided time off from work with pay to take either the E.I.T. or P.E. exam. The company will assume payment of registration fees for all regular engineering professionals.

Professional/Technical Society Plan
The company encourages active participation in technical and professional societies and will pay annual dues to two organizations for each professional employee. Time off may be arranged to attend local meetings of particular interest and benefit, and if the meeting is held in conjunction with a meal, the company will pay for the meal.

Paid time off and certain expenses may also be granted to individuals wishing to attend regional or national meetings dealing with subjects that will benefit their professional development and enhance the company's reputation.
